Telstra Health has introduced Australia’s first Fast Healthcare Interoperability Resources (FHIR) native virtual care solution – Virtual Health Platform.

Virtual Health Platform is a secure, scalable and interoperable virtual care solution, which draws on over a decade of experience in implementing and scaling virtual care solutions.

Built with FHIR at the forefront, Virtual Health Platform removes the intermediate step of translating data to a standard format and simplifies healthcare interoperability. This helps to significantly reduce implementation time and enhance overall clinical efficiency.

The platform provides a digital, automated and streamlined process for creating improved and more transparent remote healthcare delivery. The platform can be easily adopted by healthcare services of all sizes and locations and can be customised to support a variety of use cases.

Telstra Health’s Virtual Health Platform offers a secure gateway for patients and clinicians to access patient information and clinical data in near real-time through the MyHealthAccess patient portal and VirtualHealthPortal provider portal.

The cloud-first platform allows data to be both stored and exchanged using FHIR within a Clinical Data Repository. This also enables seamless integration with an Electronic Medical Record (EMR), Electronic Health Record (EHR) or other clinical systems.

The evolved platform leverages Telstra Health‘s existing virtual health monitoring capability, which empowers patients to monitor their health vitals through health monitoring devices and by completing Patient-Reported Experience Measures (PREMs) and Patient-Reported Outcome Measures (PROMs) within the MyHealthAccess patient portal.

Patients can also view their active or previous care pathway activities, including results of completed activities. Evaluation data has shown a high compliance rate among patients for completing activities for monitoring their health, with up to 90 per cent of patients uploading their health results to the MyHealthAccess patient portal*.

The intuitive MyHealthAccess portal also provides multi-channel communication options, patient education materials specific to each patient and their health conditions, and branding tailored to each healthcare provider.

The Virtual Health Platform’s VirtualHealthPortal is a comprehensive provider portal which can be made accessible to entire healthcare teams. Through a Risk Rated Dashboard, which can be tailored to the provider, clinicians can view their entire patient cohort and patient profiles, enabling them to easily identify deteriorating patients and intervene in a timely manner. This can also help to reduce the risk of hospital re-admissions.

Furthermore, the VirtualHealthPortal provides clinicians with customisable templates for reducing time spent on administrative tasks and creating clinical efficiencies.
