Driving patient-focused care for enhanced healthcare outcomes

As part of our commitment to improving lives through connected healthcare, Telstra Health is working with health organisations and funders to integrate clinical information around a patient to support a more patient-focused health system.

Our health information platforms and registers provide a secure and robust way to connect patients’ health information across multiple providers in near real time, across whole populations and sectors. This enables clinicians to have a more complete view of their patient’s health information, supporting improved quality and safety of care.

A single digital platform to support population health programs

Telstra Health’s Population Health Platform is designed to fully support preventive health, chronic disease management and population health screening programs, enabling a single electronic record for program participants.

The platform is configured to operationalise clinical pathways, from initial invitation through to diagnosis to ensure participants receive accurate and timely diagnostic services.

Our solution supports preventative health programs for entire populations by:

  • Connecting patient information with the people that need it across primary and community health, hospitals and aged care settings.
  • Supporting decisions on effective resource allocation to support better population health outcomes.
  • Integrating with relevant elements of the healthcare system to show near real-time comprehensive health information and highly customisable reporting, to allow data-driven decisions to be made.

A trusted partner of governments and healthcare organisations

National Cancer Screening Register (NCSR)

The National Cancer Screening Register (NCSR) is a world-first, digital platform developed to underpin the Australian Government’s National Bowel Cancer Screening Program (NBCSP) and National Cervical Screening Program (NCSP).

The NCSR has been designed to enable a single electronic record for each person participating in the programs, and provides a national electronic infrastructure for the collection, storage, analysis and reporting of screening program data.

It is the first digital health system in Australia to comprehensively link all relevant population screening stakeholders, including the Commonwealth Government and state health authorities, GPs, specialists, public and private pathology services, and the participant.

The NCSR supports the end-to-end operations of the screening programs by:

  • Inviting and reminding eligible people to participate in bowel and cervical screening and supporting clinical follow-up for participants with screen-detected abnormalities.
  • Generating comprehensive data to inform policy and improve screening program quality and service delivery.
  • Providing healthcare professionals better access to quality health information and making it easier for program participants to take control of their health.
