Key patient information such as demographics, clinical alerts, allied health referrals and discharge planning is displayed at a glance. Staff can easily see what actions are outstanding to progress the patient in their hospital journey:

●      Waiting for admission from the Emergency Department: Ward staff have visibility of expected ED patients, this can encourage the wards to pull the patient from ED rather than ED having to “push” the patient to the ward.
●      Waiting for admission to ward post-theatre: Ward staff have near real-time visibility of the status of their expected patients; ‘in theatre’, ‘in recovery’ and ‘ready for ward’. Nursing staff can receive automatic alerts via SMS, pager or email to inform them when a patient is ready to be collected.

●      Waiting for transition from Intensive Care Unit to a ward: Ward staff have visibility of expected transfers from ICU. This assists wards to prioritise this transfer and prepare for incoming patients.

●      Waiting for discharge: Clinicians have visibility of discharge planning activities and outstanding tasks that need to be completed to prepare the patient for discharge.

  • Improved visibility of outlier patients both at a ward and whole hospital level. Automatic flagging of outlier patients via PFM allows for staff to plan and transfer outlier patients back to their home ward. 

  • Reduction in time for Allied Health staff spent on administrative tasks. Through the use of digital referrals, staff can be alerted of new referrals either via SMS, email or pager removing the need for paper-based referral processes. Staff can create electronic worklists to better manage their workload and use reports to capture key performance information such as number of referrals, time taken to action and referrals created on the day of discharge.  

  • Reduction in time for infection prevention nurse consultants spent on administrative tasks. Patients in isolation can be flagged and automatic notifications can be triggered to notify the nurse consultant of new referrals. 

  • Increased compliance with patient information being updated in the hospital Patient Administration System (PAS) in real time. Patient Flow Manager integrates with the hospital PAS, patient information must be updated in real time for Journey Boards, handover sheets and the Bed Management Dashboard to reflect current data.
